Select a date, then a time. The booking defaults to 30 minutes but you can change the duration to 15, 30, 45 or 60 minutes (1 hour). 

Use the drop down menu to let me know what you would like to discuss. Types of appointments include: Initial Consultation, Medicare, Health - Covered California, Dental & Vision, Life Insurance, Long Term Care, Home Care Plans, and Employer Sponsored Health Insurance.

Choose: Phone Call or Zoom Meeting then fill out the rest of the form. You must click the green button that says "Confirm Booking". You will be directed to a confirmation page and receive an email confirmation as well.
